To make sure that I can provide you with the best and safest services possible, please download and complete these forms. You can then scan and email them to me prior to our first appointment or have them available for me at our first appointment.
All services will begin with an initial assessment, which will include a review of the completed paperwork and a series of functional movement, strength and endurance activities. This initial assessment typically lasts around 90 minutes and is designed for me to get to know a little bit about you, your health and your workout history.
This assessment is then followed up with a subsequent appointment where we will discuss the results of the assessment, and to create a plan for moving forward. Click HERE to download forms.

PERFORMANCE COACHING
Improve physical activity and sport performance by improving how your body moves.
EXERCISE PRESCRIPTIONS
Workout on your own, with a written plan on what to do, based on your current health and fitness level.
SEMINARS AND WORKSHOPS
Discuss various health and fitness topics, and participate in an interactive, learn-by-doing session.
PAIN MANAGEMENT
Exercise instruction for chronic joint and muscle aches and pains and to help with recovery after an injury, surgery or physical rehabilitation.
